i choose this at level 50 but the description says for ever 100 points of karma you have you gain 10 health (i gained 90) im unsure wherther it only counts the karma you had up to that point or if you gain more karma you gain more health, because i already got back very good karma for killing fiends, ghouls, and powder gangers and i still only have the +90 health buff

only counts for karma gained after the perk is taken.
