Trying to create new hat, but it gets an odd angle

I'm currently running through the desert with a cowboy character, and I'm annoyed at how quickly these flimsy cowboy hats get shot to pieces. So I thought I would make myself a hat with some damage threshhold (so I can have it repaired by vendors), and while I was at it some luck as well.

So I went into the GECK and

1) Made an Object Effect that gave 1 perception and 1 luck
2) Went to edit the desperado cowboy hat, gave it a 1 dt, 500 health, added the object effect and renamed the hat and the base id, and saved it as a new form. I put the hat into the safe at the novac motel.

I did not change anything else.

Now, I went and fetched my hat and everything would be perfect if not for the fact that my cheek is now wearing a hat! Yes, when I put the hat on, it's tilted 90 degrees so that the hat defies gravity!

How can this happen? I even deleted the esp and repeated the whole process and still the same result. What could I be doing wrong?
