I went up to report to general Tullius to be prmoted to Praefect. I went inside and quick saved, and went on to talk to him. I got my reward which was a shield of something, pretty good.
A little later I was out in the wild and got killed, so I reloaded the game to where I had just saved (at Tullius)
but this time his reward was a normal steel shield, same as I was already wearing. Reloaded again and again gave me the same shield.
Same sort of thing happened at a trader, I sold some stuff and she had plenty of coins. Killed her for the lulz and reloaded the game. Went inside and this time she had 0 coins.

Is this known?

Tried restarting the game itself, seems to have worked.
