I have the Xbox one version of Fallout 4 (Love the game by the way!) I've been having an issue with the vats system. I will run into an area and see for example a Rabid dog. I press the Right Button and select to shoot at the body of the target. Once I hit confirm to do it, it's very strange but it will spawn me in some random location about 10 miles away from where I was originally. This happens probably 2 out of 10 times I use the targeting system to kill something. It's not game breaking or anything but it is very annoying when it happens while I'm on a quest.

It first happened in Concord while I was in the power armor taking my team mates to sanctuary in the very beginning of the game.
