The first free Synth I met (Harkness), the doctor who gave him a new identity said something along the lines of having artificial parts, since he gives you one of them (it had been extracted during surgery, part of his cognitive array) to fake his death.
Which makes me think they might have artificial brains and the rest of their body is organic.
However, Kellogg mentions something about them being faster and stronger, so they might have microscopic actuator tissue embedded in their nerves and muscles to achieve this.
Spoiler If this is what they did to Shaun, that's quite creepy.
Do you think anyone would willingly volunteer to have their brain removed to replace it with a neural net module?
FYI, that technology does exist in the Fallout universe, the ThinkTank (Old World Blues DLC) did it to the Courier once, but they were fortunate enough to get their brain returned!
There's a show I've been watching called Lab Rats which features constructs similar to Synths.
They're organic humans, but have had bionic implants installed in them.
The technical schematics show that they have artificial eyes and artificial ear canols.
Their implants are regulated by a microchip clamped to their back of their spinal cord in their neck.
But aside from this, the rest of them is organic, they've just been genetically modified for endurance and resilience.
Is this more in line with what the 3rd Gen Synths are like?
But judging from the 3rd gen killed by Diamond City, there are more implants inside, because they mention seeing gears and mechanics in his face after he was torn apart by gunfire.
