Ok, like damn, this is a crap bug in more ways then one. I found I believe vault 84 around the NorthWest area from Diamond City and after plugging in my Pip Boy, nothing happened. Waited....still nothing. No sounds or movement at all, so I leave the vault and move on to other things. Only ever since that [censored] buggy vault, my VATS no longer operates at all, under ANY condition!

I've done a search and other people are also having non functioning quests and vault openings and then their VATS no longer work either. Only by entering in the console command "forceenableusercontrol" EVERY SINGLE TIME do I get VATS to work again every time I load up the game. What the hell gives here?
