I would recommend saving before picking it. Out of all the VATS activated perks I have, Grim Reaper's sprint is far and away the loudest. It's probably the loudest sound in the game.

When it activates, it let's out the LOUDEST damn sound and it's painful. Even with my TV turned down quiter than I'd like.

Just be warned. I wouldn't have it if had I known.

Yeah I have the TV at a normal volume but when my character takes pyscho he screams so loud the dog bolts and the wife isn't too hapy.

Not sure why they had super loud noises for things. It is like when you switch from a TV show to a commercial and your ear drums start to bleed.

Also maxing the cap collection crap lol the blokes exploding with caps is very very loud compared to all.

If you are on PC there is a sound mod that handles the GRS. Looks like it annoys a lot of people.
