Did all those thousands of cities get wiped off the face of the planet after the Numidium was used by the 5 factions? (Even though the Totem could only be used once, apparently, all 5 factions gained usage of the Numidium to do their biddings)

I'm pretty sure they were consolidated into the 5 factions and became a part of them.

It said on the 9th there were 44 city states and on the 11th, there were only 4.

Heh, got ninja'd by the OP.

Where did it say they were city-states? They were kingdoms, and four conquered the entire region on that day.

They ceased to be independant places, and became apart of one of the greater kingdoms. Daggerfall, Wayrest, Sentenil, and I think Orsinium.

And although the borders were redrawn, I don't think the inhabitants died.

The seven major kingdoms/nations of the Iliac Bay area after the Warp in the West are Daggerfall, Wayrest, Camlorn, Evermore, Northpoint, Sentinel and Orsinium (see PGE3 on High Rock).

City-states are sovereign territories being ruled by the city in question. The cities did not disappear, only their sovereignty. Now all of those cities answer to some higher kingdom, such as to Wayrest, Sentinel, etc.

To put it another way, the same city may or may not be a "city-state", depending upon to whom it answers. If the city is the highest authority, then it is a city-state. If the city belongs to a higher kingdom, then it is not a "city-state", it is part of said kingdom (or whatever form of governmental organization rules it).
