So, kill cams would depend on the level of difficulty? I always play on adept and I get kill cams once in a while only, nothing disturbing.

The people being Spoon-fed would be the ones complaining about getting 1-hit killed by sheer epicness, and by that wanting the game to be easier by getting rid of them.
Run away from your enemies and hit them with a ranged weapon.
I'm playing on legendary with 100 health and powerful weapons (1-3 hit kills and 1-3 hit death)
I play DiD and I haven't had any problems.
Maybe you need to practice avoiding death.
How many DiD's have you done? Every time I try one I either die straight away or get too attached to my character and when they die I go into actual mourning.

Unless the AI is messed up you are already dead when the kill cam starts for your pc... and your pc has already killed the NPC if the kill cam starts.... so basically you couldn't have saved the pc in the first place. That's the way it's supposed to work, you see "the final stroke". Just reload and keep going.
Now I have had the kill cam start with 2 npc's in the fight and the 2nd is still healthy, mostly the archery one.
No, kill cams depend on the amount of damage you do versus the amount of health the opponent has left.
If you drop him far below zero, you get a kill-cam.
This can be shown through the restoration potion loop glitch, by which it is possible to create weapons that do an insane amount of damage. You will get a kill-cam each and every single time with one of those.
No, enemy kill cams ignore armour, ignore perks and ignore player input.
Once the game decides on the basis of your raw health alone that you would not survive a hit you will get a kill cam, even if you could have easily dodged, taken a potion or if your gear, enchantments and perks would have kept you alive.
Unfortunately, that seems to be the ONLY factor taken into account when archery kill cams trigger. It's nothing short of irritating to have the damn things trigger when you know darn well you missed, and sure enough, you get to watch the arrow fly by the target in slow motion, sometimes watching the arrow actually change course before missing.
