Seems like folks are having success with disabling the Avast shields but I thought I'd chime in about something I remember from Fallout New Vegas and having crashes and sound glitches. In your speaker settings > Advanced having it set to 192000 Hz caused problems. Changing it to 24 Bit 48000 Hz solved audio issues for many. Worth a try I suppose if your having audio issues and possibly crashes related to audio.
Im having the same issue and ive disabled my antivirus and reinstalled directx it is still persistant.
It is ruining the game for me and i cant fix it.
craeteeee if you are using nvidia try disabling avast then disable nvidia streamer service ( in windows services) make sure all your drivers are uptodate . Like i said before when i installed to my ssd the game ran much smoother, maybe worth a try
Also make sure you have the patch xD
I am having similar issues (when speaking to someone their voice is louder, when cursor is about 45° of; at dialogues no sound at all). lowering video quality did not help as well as disabling kaspersky or deactivating kaspersky.
Truth to tell I can not tell if I have this patch though. Is there a way to see what version I am running ? And if he patch is still beta I would welcome some advise to install it. The manuals I found so far won't work for me as I don't have the "beta"-tab - instead there is a "dlc"-tab.

I have tried some ini tweaks and none of those work.
I was having the exact same issue as the OP, I too use Avast and once I disabled it I was able to play for over an hour without a problem; first time ever!
I have tried everything and it still persists.
