Weird fps issues

So before I start just going to post some specs:

Win 7 32-bit
GTX 460 1GB
AMD Phenom II X4 @ 3.8 GHz
4GB 1600 mhz ram

I've edited the ini file for the multicore fix (still crashes sometimes) and have done the ffdshow audio codec fix. I run the game completely maxed out with no problem except, fps fluctuactions. When outdoors, I manage 60+ FPS but it dips to 40 at times; normally this isn't a problem but it literally slows down everything from character movement to firing, reloading, NPC movement, NPC facial expression during dialogue, etc. The opposite happens when I'm indoors, my fps gets boosted to 140+ and thus my character runs so fast that its hard to control (this is even worse with lock picking as the bobby pin starts flying around at supersonic speeds). I have the Steam GOTY version (vanilla and un-modded) that has been patched to the latest version, my graphics/mobo drivers are all updated as well. I haven't checked the temps of my card or anything but I highly doubt my GPU is the problem seeing as my FPS isn't low.

Does anyone else get this? Its a real let down, seeing as I played Fo3 seemlessly on my very bad laptop on the lowest settings.
