Actually, this is a good question. His name is not mentioned in game, nor in the Wiki (Despite a rather complete history.)

I guess my riddle will remain unanswered.

I just think its odd that the Followers and the NCR call him "Caesar." That's like Americans and Brits in WWII calling Germany's leader "the Fuhrer " - they'd just call him Hitler or Adolph. They wouldn't use his 'kingly' title.

And the Followers must have known Caesar's real name. . .

Meh, I'd say he looks like a Bent-?ke, Bengt-?ke Ljungberg.

Dunno if we need his real name, each Caesar will become something else, a god amongst there people, with their former name forgotten.
But if we need to slap a name onto him then I guess Julius would be the most fitting one.
