I think you have a crippled limb. A stimpak or doctors bag should fix it.

Limb damage, it means you have a broken/crippled limb, if on Casual Mode, apply stimpack to the limb from the Pip-Boy Status screen, if on hardcoe, do the same but you have to use a Doctors Bag instead of a Stimpack, or you can talk to a doctor for a full heal for 100 caps or so.
