Worst thing? Finished the game with the Institute. Never intended to do so, but I realized that I had done one quest too many and had reached the point of no return. Forced into an endgame, I decided to see their ending, knowing that a new playthrough was about to begin.
Whacking a certain BOS member in the back of his dome while he was knelling. Felt a little nasty, but orders are orders.
Put about 3000 rounds into Strong once. That guy really annoys me. No milk of human kindness for that mother@#!$&&. Started my first play through and ended up following the institute because a mother will do anything for her son. Ended up threatening the wastelands, killed off the BOS, and Railroad. Racked up quite a body count on that first play through.
Probably intentionally starting a war with the POS BOS when I was about level 5 or so. I had just found the Fatman and wanted to see it in action, so Danse got one in the face...then got another because the first one didn't quite finish him off.
Come to think of it, that might've been the best thing I've done. They are, after all, nothing more than raiders in fancy power armor, with a superiority complex. TBH, they ain't that tough.

But you don't understand. Those squires were the best. An eager little girl accompanied me on a mission to clear the ghouls out of Hubris Comics, and on the way I decided to give her a treat and kill Swan with her. And now... thank the stars this game doesn't have nightmare cutscenes when you sleep.
Well, nothing on purpose per se. I do have the occasional misclick between enter conversation and sandman killing (E and R on the PC). Poor Tinker Tom have been killed this way numerous times Maybe his own fault, snoozing the night away like that, pffttt
- Killed a cat
- Sold Billy to a slaver
- Blew up Prydwen which means the squires died too
- Killed a settler settlement because the constant raider/supermutants spam quests annoyed me
And he calls me "babe"...in front of everyone so they all know
Robbed Trish with Henry Cook then killed her after promising to let her go. Convinced Malcolm Latimer that Marowski killed his son, Nelson. Then I took blood money from Latimer to kill Marowski when I was in fact the person that killed Nelson. Oh and then I convinced Colette that Paul Pembroke's soft ass killed Henry Cook when the truth was the Henry was alive, but decided to skip town.

I was going to do this but just after the conversation with the slaver started my companion aggroed the gunner camp right next to that house so I ended up having to kill everyone. Do you only get to sell the boy or can you turn over the entire family? That quest was just to over the top for me.
I killed the guy right after I refused to sell the kid, but then when I found his family they thanked me for saving them all from the guy, I think he is supposed to arrive at the house and offer to buy the entire family. Next time, that kid is getting sold, I don't have time to babysit every single sad little person in the wasteland.
Only the boy. If you sell the boy you can't interact with the family aside from generic " hello's " and what not.
But if you don't sell the boy, and bring him home, then I think you get a second offer for the whole family, if you leave the guy alive after the first time.
Killed everyone in Covenant. Everyone.
It occured to me that this may spoil something for someone and I don't want to be that guy.
Having to shoot down a BoS Vertibird BEFORE officially severing ties with them, because a quest NPC kept wanting to attack it instead of DOING THE DAMN QUEST.
