I can hear him breathing when i approach the front door (outside) but he must be stuck inside the house or something. Any ideas?

Use Aura Whisper or Detect Life. You will see his "ghost" standing at the front door.

Any way to fix it?

There's nothing to fix, really. He will reappear in the house or the cellar soon enough. Don't worry about it, he is not stuck there.
