In mine it shows off the true design of them.
Since characters are forced to use what Gamebryo's character creation allows means that a lot of characters couldn't have their intended design, but the concept art shows their original character design.
I believe that those are more true than the final design. (Varies from game to game depending on concept art, engine and graphics.)
I have played plenty of games to know that characters in-game look like their concept art only by a little bit of a margin, so i have learned to not trust the stuff till i see the characters in-game look like their concept art, which rarely happens.
you my friend need to learn that concept art is just that, a concept, and nowhere near the final rendition of the character. Concept art is not really lore for the game world in which the character or place appears.
