1st Playthrough, Kill everyone.
2nd Playthrough, Destroy Everything.
3rd Playthrough, Deforest everywhere.
4th Playthrough, Do all of the Above.
So will there be destructible environments?

Sadly, I do not think they will do that. It would be cool if you could have that sort of effect on the world, but I think most developers would be so fearful of "very stupid people" destroying things, but THEN complaining the missions do not work, the NPCs never show up, etc. The other problem is that would be a very complex leaving little time to make a GAME instead of just a sandbox.
In your scenario, you want to kill everyone, destroy everything, and raze the whole world, right. Sure, that sounds very cool, but wouldn’t you want the world to react to that as well? I know once I killed a village of people I would want a militia force organized to hunt me down. Were I to fire to a whole forest, I would want people from towns beating down the flames, cutting fire breaks, and mages casting water or rain spells? I would want soldiers combing the road for me if I wiped out a village! It is not really a game if there is no consequence for your actions in the world, so I would want big consequences for big actions. In addition, yes, if someone killed me for a big infraction like that I’d want it to be a game ender. Kill a few people or raze a few homes…that part of the game…kill everyone and everything the game should say, “Fine, the gloves are now off, but let’s see how long you last in a fair fight with guards after you and the NPCs undoing your misdeeds.”
