Restoration and Conjuration obviously
Destruction and alteration sound good too. I'd imagine something similar to a Witchhunter. Maybe add a one or two handed skill instead of archery though.

Weapons: magical staves and forsworn weapons only.
Apparel: furs, hides and forsworn armour only. Masks allowed. Amulets allowed. Rings, maybe. No shields, circlets or other headgear.
Magic: Illusion, alteration, conjuration. Restoration, healing only. Destruction, low-level only.
Scrolls: none.
Potions: yes, but keep to mostly elementary effects (it's at your discretion what is 'elementary' and what is not)
Skills: Alchemy, one-handed, speech, enchantment, alteration, conjuration, illusion. Maybe sneaking.

Uhm, yeah.

Have fun.

I think witch doctors would delve into poisons more than potions.

Yeah, I'm just going by the in-game 'Potions' tab, which covers both potions and poisons. I think both of these should be pretty basic, because the witch doctor isn't an assassin or apothecary- the witch doctor is basically just a creepy hick.

That would be my witchdoctor class,
conjuration-mostly reanimate and bound weapon if you want, lots of soul trapping
restoration
destruction
one hand
archery
enchanting
and or alchemy
to the one who said creepy hick, or a really hot chick

Mm, I don't agree ... Bows just don't fit with the words 'witch doctor'. I'd make an exception for Forsworn bows, though.

I have a Shaman Nord that wields a staff and a sword and raises the dead to fight beside him. He wears fur armor (shirtless with the "cape") Stormcloaks General head gear and gloves. Looks like a crazy old bum with a bushy beard, dirty and scared, and blind.

Ive had a witchdoctor since Morrowind. In Morrowind and Oblivion, they play like this:

Custom 'curse' spells, followed up by a rudimentary dagger with an accompanied poison. You can also use the 'tribal' looking staves. Since you cant create spells anymore, nor can you really cast a 'curse' spell since there's no more weakness, this strategy will have to change up a bit for Skyrim. In Sky you could actually reverse the roles and have the poison be the 'weakness', or 'curse' followed up by a spell, or a staff cast (i e hags wraith is aesthetically appealing for a witchdoctor).

As for race, I always go Argonian, just because it makes sense to me. http://i208.photobucket.com/albums/bb319/joniscrazy/Argo.jpg

So for Skyrim, it would be:

Forsworn headdress, or no head wear, to show off Argo spikes and, or feathers.

No 'armor', since I'm going for the tribal loincloth look.

Bracers or gauntlets that fit, Stormcloak, fur, etc.

Same with boots

Tribal looking dagger

Tribal staff like hags wraith.

I suppose everyone is entitled to there version of the witchdoctor, but i digress for an official class probably not
