This was with mods running, but now it is happening even though I completely uninstalled Skyrim from Steam and emptied the data folder.

I've tried no mods running, I've tried with the Unofficial Skyrim patches (all four of them), and nothing works.

Every time I face a female NPC she will occasionally use the male combat sound, not matter what I try, it happens.

This bug only occurs when I play as a male character. When I play as a female character it does not happen.

This occurs with new games.

Is there a fix for this?!

If you have tried all that then it sounds like the problem is with your savegame. Or did you start a new savegame also?

Yeah, this is with new savegames.

You said you used mods? Have you ever added anything to the Data/Sound folder?
