When I casted huge destruction spell against Lich Lord when I first time encountered it.
Spell was reflected back to me and I was dead before I was able to counter it.

Thats one I remeber.

The 'gas chambers' in Ayleid ruins. Didn't know what the hell was going on the first time that happened.

I was in an Aylied ruin (Ceyatar or something) and I was mid-battle with a Dremora; I was trying to corner it backwards through this hallway. Eventually I killed it but after the fight with so many other Daedra, my health was low (I RP so can't just drink potions out of nowhere). But alas, I was happy with my victory.

I stood there on the spot and did my 'victory dance'- basically just jumping in the air. All of a sudden, a pile of rocks and boulders fell from the ceiling and straight onto my character. Needless to say, he was crushed to death.

My worst death... comes from being over-confident after playing for so long. :) I had been on a gate-closing spree, like you do. I do different things in different gates to amuse myself, in this case, although I'd battled a few rooms of enemies, I decided just to sprint for the Sigil stone and not bother with the high ranking dremora nearby. I misjudged either close he was and got knocked down before I could get to the stone. My toon gets up, backs up a bit... aaaaand, I am focusing on one enemy, not noticing a second guy had made it up there - I get hit from both sides and I had moved too close to the edge of the platform- I went flying into the pillar of flame. which is like 10K hit points. I may still have a screenshot somewhere.

Similar story - I was making a Gate run, training enemies all the way up to the Sigil Stone. I grabbed the stone and jumped down - only to get hit into the pillar of fire. :o

Yup very cool.
I did a reaaaaalll stupid death once, when I first started I decided to go for an underwater swim see how far down it goes got quite a way down before realizing I would not have time to get to the surface, no potions or scrolls.
death by stupidity I think that is called.

The other night, I was fighting a particular named Lich in Anvil. Hit him with a custom spell which included paralyze, among other death-dealing sundries. He reflected the entire spell, and I collapsed in a heap.

After taking a long break from Oblivion, I came back to it and just started with a save I had at the sewer exit. Walked outside and was just standing there when I heard the danger music kick in. I looked around and saw a mudcrab heading towards me. And only then realized that I had completely forgotten how the controls worked. So while I madly pushed keys and tried to get a weapon equipped, the mudcrab killed me.

I use a mod that restores enchantment charges over time (though very slowly) One time I was in the middle of an Oblivion gate and I needed to recharge my sword and I'd already cleaned out the area-- or so I thought-- so I decided to take a break. I left my character standing in a hallway and went and got something to eat. When I walked back into the computer room, I could hear a familiar screeching sound. I rushed over to the PC just in time to see my character killed by a Clannfear Runt.

Ah yes, the infamous spike trap. I think we're all guilty to that old thing.

I just remember when I got killed by a Mudcrab early on in the game. I had killed those Bandits at Vilverin and then one of those pesky buggers came up behind me and nipped my leg. Due to my low health I died. For about 30 hours worth of gameplay I was scared of Mudcrabs, thinking they were some kind of super-enemy.
