There was once a movie planned to be based on Fallout and a leaked script states that a Vault-Tec executive fired the first nukes to make his vision of nuclear holocaust come true. The movie plans were however scrapped and no canon source has ever stated reliably who fired first.
But who knows, maybe Vault-Tec did fire first.
Maybe it's never been confirmed becaused it fits almost too perfectly.
It would fit better if the tension between the superpowers started to lessen, but then suddenly there was an attack. If the hostilities continued the escalate there would be no reason for vault tec to do anything, just wait for the inevitable. The only reason vault tec would have to initiate would be if it looked like the superpowers were starting to work it out and back away from war. Anyone have any idea how tense the situation was leading up to the war?
Super intense. And it was only ramping up, with China invading Alaska, and the U.S. deploying Power Armor for the first time onto the field. Limited resources were draining fast on both sides, and the U.S. was rushing solder-carried nuclear launchers into the field (The Fatman) to even the odds.
Sounds like fun. I wasn't exploring vaults in NV because I thought they have to be really boring. Maybe I will play Fallout 1,2 and enter all the vaults in Fallout 3,NV
As has been pointed out, there was no reason to do so on Vault-Tec's part. China and the U.S. were both hovering over the launch buttons anyway. And Vault-Tec's experiments make the MOST sense if no real nuclear Armageddon ever occurs. All they needed to do was trick people into reporting to the Vaults.
Besides, Vault-Tec wasn't prepared for the bombs anyway. There were numerous issues in multiple vaults where all the staff were unable to report in time, and the experiment populations weren't all in place, or there was missing equipment. If Vault-Tec was secretly responsible for the bombs, they would have prepared their Vault staff a little better.

Vault 75 can be found on the east side of the map about in the middle (north/south). To get inside you need to find the Malden Middle School, the door is located in the rubble.
If you were lucky enough to get into a control Vault, life was pretty good.
Or one of the vaults with some of the more begin social experiments. Like Vault 15, or even Vault 101. Its the ones like Vault 87 which were fates worse than death.
