Every time you see a door (IRL) you groan and say "Crap... another loading screen..."

I did that once... in front of one of my friends xD

When you get tired of it and start aimlessly hacking and slashing, and not exploring properly. Going to place A, retrieving item A, and returning to place B to return item A. Without fully poking around the place, checking chests, and looking in places you don't necassarily have to as part of the quest.

Seriously, it happened to me the other day, and i was surprised....i had to leave it for a few days and come back to it.
