Your Biggest "Facepalm" Moments

I hate Nick Valentine... I'm shooting at Super Mutants from inside a bathroom. Nick Valentine decides to run INTO the bathroom just as a Supermutant throws a grenade, preventing me from dodging said grenade. Also, not quite as deadly, but I'll be firing my combat shotgun at vermin, and Nick all too frequently decides to jump in front of my gunshots in order to pistol-whip said vermin, taking the full brunt of my shotgun blast.

I'm in a radioactive hotspot, and my companion decides right at that moment that it's a good time to talk about our feelings... By the time I untangle myself from the conversation, half my health bar is red. I've had the same thing happen just as a big battle is about to descend upon us. I've bled to death while some idiot companion decides that he needs to get something off his chest at that very moment. You have a small window to extricate yourself, but after that, it's all automated, and the worse the sounds of combat (or radiation) gets, the more it seems like the whole thing is taking place in slow motion.

Feel your pain. My game TV wears a post-it note - 'FFS save regularly.' Not exactly ideal.

As I move on I also delete old saves.

I still remember Cait telling me all about her drug problems when the two of us were standing in the middle of the Glowing Sea. The very same place where not just a few minutes before she was complaining about all the rads I was taking in. I was lucky I was wearing the hazeman suit and had popped in a few Rad-X.

COMPANION:

“I'd like to tell you something that's very deep, long, and personal, something I hold near and dear to my heart.”/“I want you to be my personal psychologist while I tell you my sob story.”

They tend to do that. I had Curie try and tell me all about becoming human when we were in the middle of fighting a massive wave of gunners.

I think the worst was when I first started playing and couldn't find any bobby pins, then found 5 and was so excited, then wasted all 5 and didn't open the safe because I didn't fully understand how to open them...

Also I picked up a legendary missile launcher (forget which one but it was a good one) I gave it to macready, then started to run down a hall, a goul came out, and he blew us both away. I did the level again and couldn't get the same legendary weapon.

Finally, the numerous times the game decided to just stop or freeze and an error message appeared on the screen...that was a big one for me.
