Any time I try to play Audio Log 12, my game crashes. It crashes the moment i click on it in my inventory. No other audio logs, from zeta or anywhere else, have this problem.

Get the code and remove it from your inventory, then put back on.

Use the list of DLCs in the launcher to figure out if the prefix needs to be 02, 03, 04 or 05

player.removeitem code 1

player.additem code 1

See if the replacement has this issue. I don't think you need to play the logs to get the achievement, just find them.
